of service for all elective services, inclusive of prior debt(s) owed

unless payment arrangements have been approved by Patient Financial Services.
If you do not have insurance, you are responsible for paying the entire
bill. Our Patient Financial Counselors can help provide financial counseling
and advice. Please call 1-800-645-0441 for more information.
Arrangements should be made prior to your admission date for payment of
all costs not covered by your insurance. If you are admitted in an emergency
situation, a representative from our Financial Counseling team will meet
afterward with you or your family to discuss financial arrangements for
any costs not covered by insurance.
