Saint Clare’s Health System is working with Jersey Health Connect, the largest Health Information Organization in New Jersey, to implement technology to enable hospitals, physician practices, labs, and other ancillary practices to securely exchange clinical information and improve coordination of care for patients in the communities we serve. An online tool connecting you to healthcare providers participating in the Jersey Health Connect network. Saint Clare’s Connect enables you to:
- Access your medical records from participating facilities, not just information from procedures performed at Saint Clare’s

- Request lab/test results
- Manage and document your prescriptions and overall healthcare
What is a Health Information Organization (HIO)?
Healthcare providers (doctors, hospitals, labs, pharmacies, etc.) sharing healthcare information through a secure, online network.
What is the name of the HIO that Saint Clare's belongs to?

What are the benefits to patients?
Providers can quickly share medical data and patients gain secure access to their medical information.
Is the medical information secure?
Yes. Patient data is strictly regulated, State and Federal laws set strict guidelines for protecting patient privacy. Jersey Health Connect has internal requirements above and beyond those set by law.
How does participating improve quality of care?
Instant access to medical records can help prevent potential medical errors and reduce unnecessary testing or treatments and enables the patient to actively engage with providers to manage healthcare.
Is there a cost to use the patient portal?
No, this is a free service to allow you to access your health information.
How do I opt out?
We are required to send information to our Health Information Organization to follow New Jersey regulations, therefore you are automatically opted into the HIO. When you are registered at Saint Clare's, you will be asked to sign an acknowledgement form (forma de reconocimiento) to make you aware your information will be sent to Jersey Health Connect. In order to opt out, please find the opt-out form and instructions below:

*PLEASE NOTE*
- For patients 11 years old and younger, the Authorization Form does not need to be completed. Saint Clare's will verify that the parent requesting access is listed on the minor's hospital record and if the information matches, we will accept the request.
- Due to New Jersey privacy laws, health information for minors between the ages 12 through 17 are not shared with Saint Clare's Connect, so you will not be able to access their information through the patient portal.
